Job Description
We are currently recruiting for 2 community care assistants in Hammersmith, This is a flexible role with varying shifts up to 36 hours per week over 7 days.
Private cars cannot be used in this role. Travel around the borough can be done on foot, cycle or public transport.
(£19.45umbrella) Going up to (£23.64 umbrella) after 12 weeks.
Double pay on bank holidays.
Role-
Travel around the borough to residents homes to give practical assistance to help individuals remain in their own home.
To provide a high standard of reablement care and non-invasive health treatment for clients referred to the Community Independence Service.
Supporting clients with personal care tasks. This may include activities such as assisting with continence care, washing, bathing, showering, washing of hair, shaving, caring for teeth; transfer from bed to chair and use of special equipment such as bath boards and raised toilet seats; assisting users to dress and undress.
Supporting clients with non-invasive health tasks. This may include activities such as application of support hosiery, application of creams, medication support/ prompts from blister packs, pressure ulcer prevention, changing catheter bags, testing urine and observation monitoring.
Supporting clients with practical care tasks. Encouraging clients to manage their practical care such as general housework, laundry, making and changing beds and assisting directly when necessary. Encouraging clients to prepare their own nutritionally balanced meals in accordance with dietary requirements and assisting directly when necessary (all types of food).
Supporting clients to maintain and develop social contacts. This may include activities such as advising clients about local leisure voluntary and community services and activities they can engage with.
Supporting clients with therapy interventions such as mobility and walking exercises.
To monitor the progress of clients, keeping stakeholders (including key case worker and clinical staff) informed of relevant information relating to progress and changes in the client.
To work in partnership with other teams and agencies within the borough to maximise reablement potential and support the transition into and out of the Community Independence Service.
To undertake environmental risks assessments and participate in client reviews as required.
To ensure that clients are safeguarded at all times within the requirements of the role raising concerns and alerts as appropriate.
To observe code of safe practice in respect of possible contact with infectious diseases.
To respond to emergencies by summoning appropriate assistance e.g. Ambulance / District Nurse / GP.
Maintain the confidentiality of clients at all times.
To be competent in the use of all relevant technological systems including e-monitoring systems and mobile technology (training will be provided for in-house systems).

To provide written and verbal reports regarding the activity undertaken and progress made against goals during supervision, at reviews and when required, utilising electronic and paper based records.
To participate in the monitoring of quality standards such as providing client feedback questionnaire forms and support clients to complete them as relevant.
To assist in the induction of new team members with particular reference to systems training, advice and guidance.
You’ll need to be physically fit and have a strong degree of common sense.
There will be training and a short period of shadowing another Community Independent Assistant
