Job Description
HCL Mental Health are urgently looking for a Band 6 or 7 (depending on experience) Community Psychiatric Nurse/Social Worker to work 37.5 hours per week covering the community of Enfield Borough.
You will work as a member of the multidisciplinary Community Rehabilitation Team (CRT) to provide effective care and specialist advice for people with psychosis and complex mental health needs and their carers, in order to support their well being and recovery.
Band 6 Pay: £25 P/H
Band 7 Pay: £30 P/H
Person Specification
Registered Mental Health Practitioner.
Dip/BSc (Hons) in Mental Health Nursing or Occupational Therapy
Evidence of further post-registration education and training or equivalent teaching and assessing qualification.
Minimum two years post registration experience, one of which will be as working with people with complex mental health problems.Responsibilities
Carry overall responsibility for the management of a caseload, acting as a care co-ordinator, as defined by the Care Programme Approach (CPA).
Deliver effective care and treatment that is carried out within a multidisciplinary framework and in collaboration with other agencies.
Involve service users and their carers in planning care that includes all of their individual and diverse needs.
Design individual programmes of care collaboratively with patients to maximise self management and social inclusion and reflect their own goals and aspirations. Review and adjust plans according to changing service user needs.
Provide evidence based therapeutic interventions using developed interpersonal skills such as CBT or psycho-education.
Provide service users and their carers with clear and intelligible information which will assist them in making informed decisions about their treatment and care.
