Job Description

MMP Consultancy are currently working with a client to recruit a Rough Sleeper Support Worker based in London on a temporary basis, with the potential to go permanent.

The Rough Sleeper Support Worker role is to identify and locate rough sleepers and provide outreach support to enable then to move off the streets and into accommodation suited to their immediate needs.

The Rough Sleeper Support Worker will work with rough sleepers and provide floating support and financial assistance to help them engage with specialist services such as mental health and substance misuse, with the overall aim of sustaining them in temporary accommodation before moving them on into more settled accommodation.

Key Responsibilities:
* To engage with entrenched rough sleepers through outreach work and share information where necessary with statutory agencies.
* To develop and maintain effective relationships with vulnerable rough sleepers
* To interview and assess rough sleepers and develop tailored support and action plans
* Support rough sleepers into temporary accommodation by providing intensive one – one support and financial assistance
* Provide floating support and engage with partners such as social care, health, community safety and DWP to help rough sleepers stay off the streets
* Support rough sleepers to move from temporary accommodation to settled accommodation in the private rented sector
* Carry out home visits to rough sleepers housed in the private rented sector and support them to maintain their tenancy through advocacy, advice and referrals to appropriate agencies
* Ensure that rough sleepers are made aware of relevant provision in the area

Key Knowledge & Experience:
* A-Levels or equivalent through experience. A good standard of English
* Extensive experience of working effectively with vulnerable people experiencing complex issues in a front-line Housing or Social Care role
* Experience in a multi-agency working and working collaboratively using a partnership model to identify and implement solutions and achieve a positive outcome for customers
* Advanced knowledge and understanding of the issues affecting vulnerable people, particularly hard to reach individuals
* A practical understanding of and adherence to Health and Safety Policies to ensure personal and staff safety particularly when working alone in customers' homes
* A full driving licence and willingness to drive
* Enhanced DBS
